Scope of Service

  • Management of minor illnesses and discomforts, such as cold and flu, dizziness and headaches, stomach pain, constipation, indigestion, eye discomfort, mouth ulcer, dermatitis, fungal infections, onychomycosis, acne, tinea capitis, lower back pain, shoulder and neck pain, joint pain, knee pain, wound care
  • Home/travel standby medications
  • Dispensing of prescription medications from public/private hospitals and private doctors (Must present valid prescription from Hong Kong registered doctor)
  • Free health and medication consultations
  • Consultation and follow-up services when required for individual with long-term medications
  • Consultation on blood glucose and blood pressure related products
  • Health related seminars, workshops and campaigns

The Council has been awarded the operation service contract by the Food and Health Bureau for a three-year operation of the Wong Tai Sin District Health Centre (WTSDHC) to provide district-based healthcare services including health promotion, health risk factors assessment, chronic disease management and community rehabilitation.

 

The Core Centre of WTSDHC is located at 2/F, Kai Chuen Shopping Centre, Kai Chuen Court, Diamond Hill, Kowloon. Four satellite centres are also set up in the sub-districts of Wong Tai Sin district, which include Wong Tai Sin (Central); Wong Tai Sin (North) - Tsz Wan Shan; Wong Tai Sin (South-west) – Lok Fu; and Wong Tai Sin (East) – Ngau Chi Wan. Primary healthcare services, including health promotion, health assessment, chronic disease management and community rehabilitation, will be provided to raise public awareness on personal health management, enhance disease prevention, and strengthen medical and rehabilitation services in the community.

 

The Welfare Council will strive to establish WTSDHC as a district health resources hub through district-based approach, medical-social collaboration and public-private partnership. A multi-disciplinary network of medical and healthcare practitioners (including doctors, Chinese medicine practitioners, physiotherapists, occupational therapists, speech therapists, optometrists, dietitians, podiatrists and medical laboratories) practising within or next to the service district will be developed. WTSDHC will also seek to coordinate and collaborate with various social welfare organisations, medical and healthcare operators and other community partners which provide healthcare, social welfare and related services in the district, and make referrals as necessary.

WTSDHC
The Health Bureau introduced the Chronic Disease Co-Care Pilot Scheme in November last year. With government subsidies, Hong Kong residents aged 45 or above and without a known history of diabetes mellitus, commonly known as diabetes, or hypertension can receive screening and follow-up services by matching them with a family doctor.

Shirley, a member of WTSDHC, was interview by the Information Services Department to share the benefits of participating in the CDCC programme. Centre Care Co-ordinator Ms Wong Tsz-wan also explained the roles of multidisciplinary team and how they help the patients.

唐女士過往因工作壓力,忽略了自身的健康情況。她的女兒遂提醒她使用地區康健中心的服務,唐女士成為康健中心會員後,在護士提供的健康風險評估中,發現自己患有高血壓及糖尿病的病徵,繼而經護士轉介到網絡醫生作進一步檢查並確診患有上述疾病。

在護士的鼓勵及安排下她參與「病人自強計劃」,當中有護士講解相關病理,物理治療師教授運動,地區康健中心一系列的服務,成功改變了唐女士的生活習慣、自我健康管理意識,更扭轉了她對健康的概念,「健康要由自己做起,愛錫家人及自己,應該及早作出健康管理,身體狀況難以逆轉,故此大家更要注重健康。」

Having been a housewife for most of her life, member Seui Hing dedicated most of her attention to caring for her family. After becoming a wellness ambassador for our centre and learning the importance of healthy eating from our multidisciplinary team of experts, she developed a regular exercise habit and became more mindful of her health and blood sugar levels. “I feel more energised now more than ever, and hope to be a living example to others in the community of what it means to take charge of my own health.”

From beneficiary to benefactor, member Mandy joined the centre’s team of volunteers as a wellness ambassador. “There are many people who need are services that don’t know what’s available to them, or are too withdrawn to reach out. I hope to show other people how our centre could help change their lives, the same way it changed mine.” Mandy hopes that with training, she would be able to impact more lives as an assistant trainer.
