The Providence Garden for Rehab Day Activity Centre and Hostel District Support Centre for Persons with Disabilities Integrated Community Centre for Mental Wellness

Service Overview

康恩園

The Providence Garden for Rehab is an integrated rehabilitation service unit under the Hong Kong Sheng Kung Hui Welfare Council. It is one of the largest rehabilitation buildings in Hong Kong. The service building is named "The Providence Garden" and does not emphasize the title of the rehabilitation building. It is community-oriented and hopes to create a life paradise full of health, peace, and grace for the service targets. By departing from the title of a rehabilitation building, we believe that members can live independently in a caring and accepting environment, and can bear abundant fruits like a fruitful vineyard.

 

PGR is the first largest comprehensive rehabilitation services complex in Hong Kong with 13-storeys in height. More than 700 residential places and 200 day places are provided, with the services of Long Stay Care Home, Hostel for Severely Mentally Handicapped Persons, Hostel for Moderately Mentally Handicapped Persons, Halfway House, Care and Attention Home for Severely Disabled Persons, Day Activity Centre, Day Care Service for Persons with Severe Disabilities, Integrated Vocational Rehabilitation Services Centre, Supported Employment and the “Sunnyway” - On the Job Training Programme for Young People with Disabilities. There are about 400 staff from multidiscipline, including nurses, pharmacist, occupational therapists, physiotherapists, speech therapist, art therapist, social workers etc. They work hand in hand to excel their professional knowledge and skills to provide quality services and living for members and their families.

 

Advocate Body-Mind-Spiritual Integration Model

PGR is based on the concept of holistic rehabilitation and advocates body-mind-spiritual development. It combines Chinese herbalist life-nurturing concept and creative art expression as an integrated service delivery model in order to bring enjoyable and healthy life for members.

 

Build up Normalized Way of Life

PGR emphasizes the concept of normalization through the empowerment model in which members share their rights and responsibilities to lead an independent life. Meanwhile, the comfortable residence design and greenery outskirt of PGR building creates home-like environment for the members. The family based and community based principle emphasizes to help members to have harmony family lift and integrate into the community.

The Welfare Council's Grace Garden for Rehabilitation - Day Activity Centre and Hostel and the Wu King Integrated Rehabilitation Services - Day Activity Centre and Hostel provides training and hostel services for people with intellectual disability to help them lead an independent life with dignity. The service focuses on developing users' potential and community integration.

H.K.S.K.H. Tuen Mun District Support Centre - The Gathering aims at enhancing the domestic living and community living skills for persons with disabilities so as to facilitate their integration into the community through the provision of a range of community support services. It also provides training and support services to the family members / carers of persons with disabilities so as to strengthen their caring capacity and to relieve their stress.

 

Scope of services

DSC would meet the needs of persons with disabilities by providing a range of services and activities which include

  • Individual or group training programmes and support services for persons with disabilities to enhance their domestic living and community living skills;
  • Social, recreational and personal development programmes for persons with disabilities;
  • Training programmes / educational courses / workshops for family members / carers of persons with disabilities to enhance their caring capacities;
  • Supportive services to family members / carers of persons with disabilities to relieve their stress;
  • Community education;
  • Drop-in service for persons with disabilities;
  • Information on community resources and referral services for appropriate welfare services; and
  • Provision of case management service including conducting need assessment, formulation of case plan and rendering counseling service to the persons with disabilities and their families and to bridging up community resources.

Through the one-stop and integrated service mode, H.K.S.K.H. Tuen Mun Integrated Community Centre for Mental Wellness - The Club provides district-based and accessible community support and social rehabilitation services ranging from early prevention to risk management for discharged mental patients, persons with suspected mental health or emotional problems, their families / carers and residents living in the serving district.

“The Club” serves the district of Tuen Mun Town Centre, San Hui, Sam Shing, Hanford, San King, Leung King, Po Tin, Kin Sang, Siu Hong, Yan Tin Estate, Prime View, Fu Tai, Wo Tin Estate, Ching Tin Estate, Tuen Mun Rural and So Kwun Wat.

康恩「樂藝園」共融壁畫計劃
會員佩玲自中學畢業後便沒有執筆畫畫,創作壁畫對她而言是個不可能的任務,不過在藝術治療師的鼓勵下,她決定一試。「我畫了一隻小鳥,想不到牠雖然構圖簡單,但當其他會員和職員為牠塗上顏色後卻變得栩栩如生,每個人也為這幅壁畫付出不少心血!」透過計劃的社群藝術活動,佩玲有機會跟會員和職員面對面或藉著畫作互動交流,從而感受到別人對自己的欣賞和接納。

Application

  • Referrals can be made by social workers and staff of rehabilitation service units to the Central Referral System for Rehabilitation Services of the Social Welfare Department.
  • Residential respite service can be applied directly at our centre or via social workers of medical social service units, integrated family services centres, special schools or rehabilitation service units.
  • Referrals of Day Care Service for Persons with Severe Disabilities can be made by social workers to the service unit directly.
  • Sunnyway - On the Job Training Programme for Young People with Disabilities can be applied via our centre's social workers.
