The H.K.S.K.H. Multi-disciplinary Outreaching Support Teams for the Elderly (Kowloon Central and Kowloon East Cluster) provide free social and rehabilitation services at private, contracted and self-financing residential care homes for the elderly in Kowloon Central (Kowloon City and Yau-Tsim-Mong District), as well as Kowloon East (Kwun Tong, Wong Tai Sin and Sai Kung District).
The scheme delivers services through a professional team of occupational therapists and physiotherapists, who conduct clinical assessments, regular assessments, rehabilitation training for private residential care homes for the elderly, and offer professional advices and training to staff from the homes and caregivers. The scheme’s social workers also provide social services to elderly residents, which addressed their physical need and enhanced their quality of life. Moreover, the scheme offers speech therapy to residents of contracted or self-financing homes.
Scope of Services
1. Physiotherapy: pain management, muscle-strengthening activity and stretching exercise
2. Occupational therapy: cognitive assessment, self-care training and proper sitting posture

Service Overview
Frail senior citizens who require daytime attention and are aged 60 or above find the services they need at the Welfare Council’s seven day care centres for the elderly. Our centres assist senior citizens with day-to-day living, socialisation, recreation and rehabilitation to help provide comfortable and familiar environments in which older adults can age actively.
Your loved one is eligible for services provided by our day care centres if he/she is
- Aged 60 or above and living within the same communities as our day care centres;
- Not receiving residential care services; and
- Deemed moderately impaired or above under the Standardised Care Need Assessment Mechanism for Elderly Services and suitable for day care services.

Our day care centres support senior citizens who suffer from moderate to severe functional disabilities and/or dementia. We ferry our senior citizens from their residences to our centres, where we help them stay as mobile as possible, develop their potentials and improve their quality of life. The Council started to promote elderly education in early nineties. In order to further promote this service, the Vitalita Academy For Life Long Learning was inaugurated in 1998. Cherishing the spirit of “Lifelong Learning” and “Productive Ageing”, the Academy strives to extend learning opportunities for the seniors in Hong Kong.

Service Overview
The eligible elderly persons may use Community Care Service Voucher to choose community care services (CCS) that suit their individual needs to support their ageing in place under the 'money-following-the-user' mode. The service delivery modes include Centre-based service (full-time or part-time), Home-based service and Centre-based and Home-based service (mixed mode).
Eligibility
Eligible participants are the elderly persons who have been assessed and recommended for Community Care Services or Residential Care Services under the Standardised Care Need Assessment Mechanism for Elderly Services, and are waitlisting for subsidised CCS and/or residential care services (RCS) on the Central Waiting List (CWL) for Subsidised Long Term Care (LTC) Services without any kind of RCS or subsidised CCS being received.

Service Overview
The Social Welfare Department has implemented the Pilot Scheme on Training for Foreign Domestic Helpers (FDHs) in Elderly Care since 2018. A professional team comprising nurses, dietitians, physiotherapists, and occupational therapists provides comprehensive training to enhance the skills of FDHs in caring for frail elderly individuals. The aim is to improve the quality of life for the elderly in the community and support their ability to age in place. Additionally, the training equips FDHs with the knowledge to establish healthy lifestyles for the elderly, thereby reducing potential risks.
The District Elderly Community Centres under the Welfare Council have joined the Pilot Scheme on Training for Foreign Domestic Helpers in Elderly Care and offer courses free of charge. Employers who are interested in our course are welcome to get in touch with our staff for more information.

Full-time service
- Attend centre for 4 days or more in a week (suitable for elders with low self-care ability and lacking the care of family members during daytime)
Part-time service
- Attend centre for less than 4 days in a week (suitable for elders with higher self-care ability and having family members to provide them with partial care during daytime)
Day respite service
- Provide short-term or temporary day care service for frail elders to allow the carers to take rest in times of need, so as to enable them to continue taking up the carers’duties
- Users of day respite service do not need to undergo Standardised Care Need Assessment Mechanism for Elderly Services or follow the referral procedures. Elders or their carers can approach the centre direct for enquiry and application

Hong Kong Sheng Kung Hui Mei Mao Keen Ying Home for Senior Citizens is a medical and rehabilitation-focused residential facility that provides a comfortable environment and comprehensive medical care for elderly with rehabilitation needs. The home offers single rooms, triple rooms, and quadruple rooms tailored to the individual needs , with a total of 54 long-term and short-term accommodation options. The fourth floor features a platform garden and a green trail for residents to stroll along, as well as a "wandering path" specifically designed for residents with degenerative brain conditions. The garden also includes a planting area for residents to enjoy the pleasures of gardening and relaxation.
In addition, the home offers stroke rehabilitation services including intensive rehabilitation treatment, medical care, psychological counseling, caregiver support, and training services over a period of 3-6 months to help patients improve their overall abilities.

Application
- Social Welfare Department will send invitation letters to eligible elderly persons who are newly registered on the Central Waiting List (CWL) to apply for CCSVs.
- The elderly persons may also approach their responsible workers or the CCSV Office for assistance.
Service users
Ordinary cases:
The elderly persons aged 60 or above, persons with disabilities, and individuals and families with social needs living in the community.
Frail cases:
Assessed to be suffering from moderate or severe level of impairment and recommended for service under the Standardised Care Need Assessment Mechanism for Elderly Services.

Application
Ordinary Cases:
- Service users or their families can approach our center for application
- Referral by NGOs, Social Welfare Department or medical social workers
- Application will be assessed by our social worker
- Applicants will be put on the waiting list if the places have been fully taken up
Mild Impairment cases:
- Elderly or their families can first apply to waitlist for the Integrated Home Care Services (Ordinary Cases) from our center.
- Our social worker will conduct assessment under a designated assessment tool
- Cases who are assessed and recommended for home care support services under the Standardised Care Need Assessment Mechanism for Elderly Services can be referred to our center via their case worker
Frail Cases:
- Referral by the Standardized Care Need Assessment Mechanism for Elderly Services from Social Welfare Department
